Position: Social Worker - Preparation to Adulthood Team
Location: Warrington Council
Hourly Rate: £30.00 per hour
About the Role:
Ackerman Pierce is currently seeking a dedicated and experienced Social Worker to join the Preparation to Adulthood Team at Warrington Council. This is an excellent opportunity for a passionate individual to support young people transitioning from children's services to adult services, ensuring they receive the appropriate care and support to thrive in adulthood.
Key Responsibilities:
- Manage a caseload of young people aged 16-25 who are transitioning to adulthood.
- Conduct comprehensive assessments to identify the needs and strengths of young people.
- Develop, implement, and review transition plans in collaboration with young people, their families, and multi-agency partners.
- Provide information, advice, and guidance to young people and their families regarding available services and support.
- Work collaboratively with education, health, and other services to ensure a seamless transition to adult services.
- Advocate for young people to ensure their voices are heard and their rights are upheld.
- Participate in multi-disciplinary meetings, reviews, and panels.
- Maintain accurate and up-to-date case records in line with statutory requirements.
- Ensure safeguarding procedures are followed and respond to any concerns promptly.
- Support young people in developing independence skills and achieving their goals.
Requirements:
- Qualified Social Worker with a degree in Social Work or equivalent.
- Registered with Social Work England.
- Experience in working with young people and transitions to adulthood.
- Strong assessment, analytical, and decision-making skills.
- Excellent communication and interpersonal abilities.
- Ability to work effectively within a multi-agency framework.
- Commitment to safeguarding and promoting the welfare of young people.
- Knowledge of relevant legislation and best practices in transition planning.
