We have partnered with one of the leading GP practices in the Reading area which place a focus on data insights, technology and clinical innovation. Ultimately, improving patient access, patient experience and health outcomes to significantly increase the quality of care for their patients.

Position Details:

• Job Title: Practice Nurse.
• Job Type: Part-time (Up to 20 hours per week)
• Location: Reading
• Salary: £38,000 - £43,000 per annum with an extensive benefits package including; 27 days annual leave (pro-rata) plus bank holidays, NHS Pension Scheme, Life Assurance, Enhanced sick pay, health cash plan, Nursery Scheme plus much more.

Main Duties:

• Responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
• Ensure nursing services are delivered effectively to the entitled patient population.
• Responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics.

About You:

• Registered Nurse with experience within a GP setting.
• Experience in chronic disease management inc. diabetes, hypertension, and respiratory conditions.
• Imms/Vacs trained, inc childhood imms.
• Experience using EMIS is desirable but not essential as full training provided.
